The short version

What we collect
Only what you volunteer. The appointment and contact forms ask for your name, email address, telephone number, and whatever you write in the message field. Nothing else on this site asks you for anything.
Form submissions are delivered to the practice so we can reply to you. That is the whole purpose, and it is the only purpose.
What we do not do
We do not sell, rent or share the information you give us. We are the sole owner of all information collected on this site, and we do not disclose it in any way not described on this page.
Cookies and tracking
This website does not run analytics, does not carry advertising tags, and sets no third-party cookies of its own.
One exception is worth naming plainly. The contact page embeds a Google map so you can find the practice. That embed is served by Google and is subject to Google's own privacy terms, in the same way that clicking any link to another website takes you under that site's terms. No other page on this site loads anything from a third party.
Links to other sites
This site links out to Google, to social profiles, and to downloadable forms. Once you follow a link away from this website, the privacy practices of that destination apply rather than ours.
Your patient records
This page describes the website. Patient records held by the practice are a separate matter, governed by state and federal law including HIPAA, and are handled under the practice's own patient privacy procedures. Ask at the front desk for those, or call (941) 927-5411.
Questions, corrections and removal
If you have submitted a form and want to know what we hold, want it corrected, or want it deleted, call the practice on (941) 927-5411 or write to us at 3800 Clark Rd., Sarasota, FL 34233. We are open Monday to Thursday, 8am to 4pm.
Changes to this policy
If the way this website handles information changes, this page changes with it before the change takes effect.
